Some additional costs may apply for E911 or extra services, but even those costs are quite low:
E911 service (required in most cases) is an additional $5 per month per physical location.
If you want to keep an existing number (this is called "porting" a number), it will cost you an additional $20 up front per local or toll-free number.
Additional local or toll-free numbers can be added for $20 per month. This includes an extra 1000 minutes for local numbers or an extra 500 minutes for toll-free numbers (total, not per seat) per month.
Calls to international numbers are billed separately as these rates change almost daily, but they are typically pennies per minute.
If you go over your included minutes, there is a small charge of two cents per minute for local numbers or five cents per minute for toll-free numbers.
Custom programming or unusual PBX setups will be charged an additional setup fee depending on the complexity of the system.

Consider the following examples:
Buy three phones for a small office and set up a web design company with a local number. It is expected that the monthly usage will be under 3000 minutes per month. The up front cost is $300 for the phones and the monthly cost is $75 for the phones plus $5 for the E911 service. Total cost over 12 months is $1260, or $35 per phone over the course of the year. If you're a not-for-profit, your monthly phone bill will be $31.50 per phone!
A grass-roots not-for-profit organization starts with five people who work form their homes. They only answer a toll-free hotline and do the vast majority of their communications via social media web sites and email campaigns. Two people are married and work from the same house, sharing a phone with distinctive ringing for each person's extension so they can tell who the call is for. They get a new toll-free number as part of the service, and expect to use all 2000 minutes of it. That's a total up front cost of $400 for four phones (since one will be shared). The included toll-free service is break even each month, and if they do occasionally make a few outbound calls, they will simply pay two cents per minute. In this case, E911 services are not required so the entire monthly cost is just $100 for the phones. A client with offices located in Miami, Chicago, New York, and Denver buys 50 phones. They add four local numbers to include the extra offices, and port an existing toll-free number to the new system. That makes the up-front costs $5,000 for the phones plus $20 for the toll-free port. Monthly costs are $1,250 for the hosted service $5 per location for E911 ($20 total), plus $20 per number for the additional offices (an extra $80 per month). That gives them a total of 4000 minutes per month of local usage plus 25,000 minutes per month of inbound toll-free, all included. Let's say they use 4500 minutes of local time per month. That results in overage fees of $10 per month. Their total monthly expenses are $1360. Including the up front costs, that ends up being just $35.53 per phone.
